Instructions

  • Open retool - CityHealth COVID-19 On-Premise Sample Collection Tool
  • Select Abbott Rapid Molecular option for Test Type
  • Fill in patient form with Chart ID, visit time and test result
  • Verify patient name and DOB
  • Click submit - once complete, this will send out a text and email to the patient with a link to the results in QR Code/PDF form
