Open retool - CityHealth COVID-19 On-Premise Sample Collection Tool

Select Abbott Rapid Molecular option for Test Type

Fill in patient form with Chart ID, visit time and test result

Verify patient name and DOB

Click submit - once complete, this will send out a text and email to the patient with a link to the results in QR Code/PDF form
